2024-2025 Lake Ontario Region (LOR) Permission To Skate

A player in the LOR Region who plans to attend the tryouts of teams in U14 to U18 (U18 AAA excluded) of a different LOR Member from which they were last registered MUST obtain this form and present it at tryouts. The “Lake Ontario Region” is an area or geographic subdivision that is made up of the following: GTHL (Toronto, Mississauga, Vaughan, Thornhill, and Scarborough), and the surrounding OMHA area including Oakville, Brampton, Richmond Hill, Ajax-Pickering and Markham (see Markham Programs Regulation E64) which is the perimeter. PTS forms MUST be acquired from the association the player was with during the past season.

2016-2017 Lake Ontario Region (LOR) Permission To Skate

A player in the LOR region who plans to attend the tryouts of a different LOR association than where they were previously registered MUST obtain this form and present it at tryouts. The “Lake Ontario Region” is an area or geographic subdivision that is made up of the following: GTHL (Toronto, Mississauga, Vaughan, Thornhill, and Scarborough), and the surrounding OMHA area including Oakville, Brampton, Richmond Hill, Pickering, Ajax and Markham (see Markham Programs Regulation E80 – E82) which is the perimeter. PTS forms MUST be acquired from the association the player was with during the past season.

This form is to be used ONLY by those players wishing to attend a Junior Hockey Tryout Camp.

2017-2018 Lake Ontario Region (LOR) Permission To Skate

A player in the LOR Region who plans to attend the tryouts of teams in Minor Bantam to Midget ( Midget AAA excluded) of a different LOR Member from which they were last registered MUST obtain this form and present it at tryouts. The “Lake Ontario Region” is an area or geographic subdivision that is made up of the following: GTHL (Toronto, Mississauga, Vaughan, Thornhill, and Scarborough), and the surrounding OMHA area including Oakville, Brampton, Richmond Hill, Pickering, Ajax and Markham (see Markham Programs Regulation E64) which is the perimeter. Team Bank Account Authorization

Teams who need to open a bank account must get approval from their association first. Use this form to request authorization to open a team bank account.

Vulnerable Sector Check (VSC) Request Authorization

Volunteers who need to have a vulnerable sector criminal screening completed, and are in a volunteer position, will need to have this form completed prior to making the request from their local law enforcement agency.
